Scholarship and youth applications

Application information is intended for authorized CMAC reviewers and program leadership. It is not intended for publication on the public website. Applicants should not submit highly sensitive information unless specifically requested by the form.

Email and newsletter communications

If you subscribe to community updates, your information is used for CMAC communications and may be processed by the chapter’s email service provider. You may unsubscribe using the link included in those messages.

Analytics and technical data

Children and youth programs

Youth program forms may require parent or guardian participation and acknowledgment. Parents or guardians with questions about submitted information should contact the chapter.
